Study Links Blood Pressure Diabetes and Smoking Control to Longer Dementia Free Life

Tuesday, August 11, 2026 · 2 sources

Adults who keep normal blood pressure, avoid diabetes, and do not smoke from middle age into their late 60s lived nearly 13 more years without dementia in an NYU Langone Health study. The work followed more than 12,000 people free of dementia at the start.

Middle-aged adults who avoided high blood pressure, diabetes, and smoking lived nearly 13 extra years free of dementia compared with people who had all three risks, according to research led by NYU Langone Health.

The study, published in Neurology Open Access, tracked 12,409 people with an average age of 56 who did not have dementia when they entered the research. Investigators checked three vascular risks: high blood pressure, diabetes, and smoking. They then followed who developed dementia over the ensuing years.

People with none of the three risks lived without dementia for about 30 years after the study began, on average. People with all three risks averaged about 17 dementia-free years. The gap of nearly 13 years was the headline finding. The researchers focused on the period from roughly ages 48 to 68 as a window when control of those risks may matter most for later brain health.

Vascular disease can damage small blood vessels that feed the brain. High blood pressure, diabetes, and smoking are long-known drivers of that damage. The new analysis does not prove that fixing those risks always prevents dementia. It does show a strong association between midlife vascular health and more years of clear thinking later on.

Doctors quoted in coverage of the study said the message is practical. Check blood pressure regularly. Get screened for diabetes. Quit smoking or never start. Those steps already lower heart attack and stroke risk. The dementia data give another reason to act in the 50s and 60s, not only after memory trouble begins.

The study has limits. It cannot assign every dementia case to the three risks alone. Genetics, education, exercise, sleep, and other factors also play roles. Still, for a 50-plus audience, the takeaway is simple: the same habits that protect the heart appear to buy more years without dementia.

The 50+ takeaway: Normal blood pressure, no diabetes, and no smoking in midlife tracked with nearly 13 more dementia-free years. Ask your doctor about all three.

What three risks did the study track?

High blood pressure, diabetes, and smoking. People free of all three from midlife into their late 60s had the longest dementia-free span in the sample.

How large was the difference?

About 13 years. Those with no risks averaged roughly 30 dementia-free years after the study started, versus about 17 years for people with all three risks.

Does this prove I can prevent dementia?

No. The study shows a strong association, not a guarantee. Other factors such as genes and lifestyle also matter. Controlling the three risks is still good medicine for heart and brain.

What should a 50-plus reader do this week?

Measure blood pressure, ask about diabetes screening if it is overdue, and get help to quit smoking if you still use tobacco. Those steps help even if memory feels fine today.
